Reopening of the "eBaby" Service on the DiiA Portal
The DiiA platform has reintroduced the comprehensive service "eBaby", allowing parents to quickly and easily apply for up to 9 state services with a single application.
As a result, the Odesa City Council Administrative Services Center is resuming the acceptance of documents for this electronic public service.
Parents can submit an application for the "eBaby" service if:
- the child does not yet have a birth certificate;
- the child was born in a healthcare facility in Ukraine;
- both parents are citizens of Ukraine.
To submit an application, you will need:
- marriage certificate (if available),
- passports of both parents from Ukraine,
- TIN (Tax Identification Number) of both parents,
- UDR (Unique Demographic Record Number) of at least one parent,
- electronic medical birth certificate or medical birth certificate (form No. 103/o),
- certificate of refusal to receive the one-time natural assistance package “Baby Package” from the maternity hospital,
- bank social account number of the child benefit recipient (the parent on parental leave),
- "PrivatBank" card,
- special account for "Baby Package" (in case of refusal from the natural assistance "Baby Package").
To receive this comprehensive service, parents of newborns must pre-register for an electronic queue.
